Below are the official hotels have been appointed that offer special hotel accommodation rates for the International Thalassaemia Conference 2008.
* Rates are subjected to 10% service charge and
7% GST.

TO ENSURE A ROOM HAS BEEN RESEREVED, ALL INFORMATIONS IN THE BOOKING FORM NEED TO BE COMPLETED WITH SIGNATURE AND FAXED TO THE CONFERENCE SECERATRIAT. NO RESERVATION WILL BE MADE FOR INCOMPLETE FORMS. EMAIL & TELEPHONE REQUESTS WILL NOT BE ACCEPTED.

All official hotels require a credit card number, expiry date and one night deposit to secure reservation. Hotel 81 deposit is based on S$150.00++. The remaining payment should be settled at check-out and will be charged in Singapore dollars. Any cancellation must be notified in writing to International Thalassaemia Conference 2008 Secretariat & Housing Bureau by
30 June 2008. In this case, full duration of room nights booked will be charged to your credit card.

Kindly note that the official check-in time is at
1500hrs and the check-out time is at
1200hrs and a 50% charge will be imposed for check-out before
1800hrs and a full day rate for check-out after
1800hrs.

Kindly note that those who will arrive early in the morning are advised to book an extra night so that you will be able to check into the room before the official check-in time.

Room bookings through airline programmes and travel agents will not be entertained by the hotels.

Please retain a copy of this form for your record.

Within five working days from the receipt of the signed hotel booking form, an acknowledgement note with a confirmation number will be issued to you via email or fax only. Please state clearly your email address and fax number in the form.
9.
As a limited number of rooms have been reserved for participants, please make and confirm your booking promptly. The closing date for hotel reservations is
15 August 2008. After 15 August
2008, the booking of rooms will be subjected to room availability of the individual hotels. The Conference Secretariat & Housing Bureau will assist in making arrangements on hotel reservations on your behalf.
