How to Register
o All participants are encouraged to visit the website: www.thalassaemia2008singapore.org/app/mainlogin.aspx for individual online registration which opens on 31 January 2008, Thursday.
o Alternatively, you can complete, sign on the PDF registration form and send back, with the appropriate payment details and all related correspondence properly enclosed, to the Conference Secretariat at address stated above. Kindly note that registration forms received without payment or credit card authorisation will not be processed. Only individual registration will be accepted (one registration form per participant). For group registrations (groups of 10 paxs or more), please contact the Conference Secretariat for group registrations details and instructions. All group registrations must be received by 31 August 2008, Sunday.
o If you have already registered online or faxed your registration form with a credit card payment, please do not mail a copy of this form as it may result in a duplicate registration.

Registration Fees
All participants are required to pay the Basic Registration Fee (in Singapore Dollars) stated in the registration form, which includes the following:

§ Admission to all scientific sessions at the 11th International Conference on Thalassaemia & Haemoglobinopathies and the 13th International TIF Conference for Thalassaemia Patients & Parents (excluding the Pre-Conference Workshops) and trade exhibition

§ One lunch and two tea breaks per full-day conference
§ Certificate of Attendance

§ Name badge, conference bag, programme cum abstract book
§ Admission to all sessions in the 13th International TIF Conference for Thalassaemia Patients & Parents 2008 (excluding the 11th International Conference on Thalassaemia & Haemoglobinopathies and the Pre-Conference Workshops) and trade exhibition

Registration Deadlines
o  Deadline for Early Registration : 15 August 2008, Friday
o  Deadline for Group Registration : 31 August 2008, Sunday
o Deadline for Standard Registration : 31 August 2008, Sunday
Confirmation Registration Deadlines
You will receive the confirmation either via email or fax only. Please include your email address and fax number clearly in the registration form. 
Please bring along your registration confirmation letter to the registration counters (located at the lobby, level 1, of the Conference Venue) as proof of your payment.
Cancellations and Refunds
Any cancellation and request for refunds must be submitted in writing to the International Thalassaemia Conference 2008 Conference Secretariat by 
15 August 2008, Friday. Refunds, less SGD 100 administrative fee, will be made within two months after the International Thalassaemia Conference 2008. 
No refunds will be made for requests received after 15 August 2008, Friday. 
On-Site Registration
For on-site registration, only cash (Singapore Dollars) and credit card payments will be accepted. The registration, tour and information counters will be located at the lobby, level 1, of the Conference Venue.
